Claims Submission Update for Out-of-State Laboratory and Radiology Providers
Effective November 23, 2026, Out-Of-State Laboratory and Radiology providers with Billing Taxonomy 291U00000X or 335V00000X must include both the Ordering Provider and the Service Facility Address on claims. Claims submitted without this required information will be denied.

- Field 17: Ordering Provider NPI with Qualifier DK
- Field 32: Service Facility Address
837 Professional (837P) Claim Submission Requirements
- Loop 2420E:
- NM1 – Ordering Provider
- Qualifier = DK
- Loop 2310C:
- NM1 – Service Facility Name
- N3 – Service Facility Street Address
- N4 – Service Facility City, State, and ZIP Code
