HCBS Provider Self-Assessment

The HCBS Final Rule is designed to ensure that all individuals receiving long-term services and supports under the 1915(c) waiver programs have full access to the benefits of community living and the opportunity to receive services in the most integrated setting appropriate.

The HCBS Final Rule is designed to ensure that all individuals receiving long-term services and support under the 1915c waivers have full access to the benefits of community living and the opportunity to receive services in the most integrated setting appropriate.

The HCBS final rule is applicable to sites for the following services for adults in the I/DD and TBI populations: adult day health, day support, residential supports (licensed and unlicensed sites), and supported employment when the only when member is working at the corporate site. This rule applies to both NC Innovations and TBI 1915c waivers; Long Term Community Supports (LTCS); and 1915i waiver services.

The following are specific services/sites that are required to meet these characteristics:

  • Innovations, TBI Waiver, LTCS – Residential Supports
  • Innovations, TBI Waiver, LTCS – Day Supports
  • Innovations, TBI Waiver, LTCS, 1915i Waiver – Supported Employment (at corporate site)
  • TBI Waiver – Adult Day Health,

All sites must have an accepted HCBS Provider self-assessment prior to beginning services at that site.

Prior to the start of service, all sites must have an accepted HCBS Provider self-assessment. For those receiving services under the TBI waiver, the site must have an onsite monitoring prior to the start of service.

The HCBS process is initiated with HCBS Change Notice Form submitted by the designated care manager to [email protected]. This form is required for the following occurrences:

  • Provider change
  • Provider acquisition
  • Change in-site address
  • New service
  • Discharge from service

Providers found not compliant with HCBS rules and regulations may face administrative action.
