The Children and Families Specialty Plan is a single, statewide NC Medicaid Managed Care health plan for NC Medicaid-enrolled children, youth, and young adults served by child welfare.

What is the Children and Families Specialty Plan (CFSP)?

The Children and Families Specialty Plan is a single, statewide NC Medicaid Managed Care health plan for NC Medicaid-enrolled children, youth, and young adults served by child welfare. The plan aims to deliver and coordinate timely and consistent access to integrated healthcare to Medicaid-enrolled children, youth, and families engaged in the child welfare system.

Starting December 1, 2025, children in the custody of the Department of Social Services (DSS), former foster youth up until age 26 and other Medicaid-eligible individuals in similar categories may be served by Blue Cross and Blue Shield of North Carolina (Blue Cross NC) via Healthy Blue Care Together (HBCT).

Will there be a warm handoff period between Healthy Blue Care Together (HBCT) and Alliance?

There will be a warm handoff between Alliance and HBCT to support a smooth transition for members starting October 2025. Please note that only member information and documents will be shared at this time. Alliance will continue to provide care management to members until December 1, 2025.

Will Alliance continue supporting members through the transition?

Alliance will continue to serve members through November 30, 2025. On December 1, 2025, Healthy Blue Care Together (HBCT) will begin providing care management for eligible members.

Will Alliance still be involved after December 1, 2025?

What happens when Alliance stops serving members who transition to the Children and Families Specialty Plan (CFSP)?

Alliance will continue to manage all existing populations not transitioning to CFSP.

Will current services transition seamlessly?

Healthy Blue Care Together (HBCT) is contracting with providers to prevent disruptions.

What will happen to youth in the Crisis Stabilization and Transitional Program (CSTP)? What does this transition look like?

Healthy Blue Care Together (HBCT) may negotiate a contract with providers who have CSTP beds to continue continuity of care.

What will the process be for members who utilize the Department of Social Services (DSS) specific Wake Flex funding?

Alliance will process DSS Wake Flex fund requests through November 30, 2025. Healthy Blue Care Together (HBCT) and Wake County DSS will determine a plan for fund use after that date.

What will the process be for members in Mecklenburg County whose room and board invoices come to Alliance for payment?

Alliance will process Department of Social Services (DSS) room and board requests through November 30, 2025. Healthy Blue Care Together (HBCT) and Mecklenburg County DSS will determine a plan for fund use after that date.

Will the regularly scheduled stakeholder meetings continue?

Healthy Blue Care Together (HBCT) will work with stakeholders regarding future meetings.
