Consent Form to be Sent Text Messages from Alliance Health

How does this service work?

Members, recipients and their caregivers who sign up for this service can message with Alliance Health through a secure program. To use this service, you must first sign this consent form. We will then send you a text message from 323-431-9301. For the first message, you will need to send your (or the member/recipient’s) date of birth to confirm who you are.

If at any time you need help, just text “HELP” to the short code, and we will send you instructions on how to use this service as well as how to unsubscribe (cancel service).

Alliance can send messages to these major mobile phone carriers: AT&T, Alltel, Boost Mobile, MetroPCS, Nextel, Sprint, T-Mobile, U.S. Cellular, Verizon Wireless and Virgin Mobile. We can also send messages to some regional carriers. Please check with us if you are not using one of the major carriers.

As always, message and data rates may apply for any messages sent between us. If you have any questions about your text plan or data plan, contact your wireless provider.

How quickly will I get a response from Alliance?

We cannot guarantee a text response time. The usual response time is less than one business day, but it may take up to a week or longer if the person the text is sent to is away. Alliance employees will try to read and respond quickly to all text messages.

This service should not be used for any medical emergencies or other matters that need a response quickly. If at any time you have an urgent question, do not understand a message we send you, or do not want to discuss an issue via text, call our office.

What if I no longer want to get the texts?

You can cancel the texts at any time by texting “STOP” to the number. You will then get a message to confirm that you have unsubscribed. After this, you will no longer get text messages from us. Your care worker can always set this up again if you would like.

Are there any risks to using this service?

There are some risks to consider before using this type of service. If you are worried about any information being seen by other people, or if the question or problem is urgent, then other forms of communication such as calling should be used.

There are some other risks to text messages such as:

  • Text messages can be sent on to other people, stored on a computer or printed out on paper.
  • Text messages can be sent to the wrong person by accident.
  • Text messages may be kept on computers or other devices even after being deleted by the sender or recipient.
  • Businesses and online services have a right to archive (store) and look at text messages sent through their systems.
  • Text messages can sometimes be changed, forwarded or used without authorization (permission) by others.
  • Text messages can be used as evidence in court with proper legal documents.

Alliance Health and the service providers of the secure platform are not liable (responsible) for delayed or undelivered messages, or for the security of the messages on your or your caregivers’ devices. We also are not responsible for any wrong information given to us, such as phone numbers.

To help keep these messages secure, you should let us know right away if:

  • You or anyone you have given permission to get messages on your behalf changes phone numbers or loses a device.
  • You think your device’s security is at risk in any way.
  • You want to add or delete someone who has permission to be sent messages on your behalf.

Please make sure you give us correct and up-to-date information to prevent your personal information from being shared with people who do not have permission.
