Grievances Submission

This page describes the process providers can use in submitting a grievance.

Alliance gives all providers written notice of the provider grievance procedures at the time they contract with Alliance.

Providers may file a grievance orally or in writing or dispute any aspect of the operations, activities, or behavior of the Alliance Health Plan except for any dispute for which the provider has appeal rights.  A provider may file a grievance in writing by mailing or faxing the complaint to Alliance’s Grievance Department.

Providers must file their grievance no later than 30 calendar days from the date the provider becomes aware of the issue generating the complaint. When acting as the member’s representative, a provider may not file a grievance on behalf of the member without written consent from the member.
