Alliance Health works to connect eligible adults living with Serious Mental Illness (SMI) or Severe and Persistent Mental Illness (SPMI) to community-based supports including safe and affordable housing. TCL seeks to identify adults living in or leaving facilities in North Carolina to help them live and thrive in community settings.
Alliance also manages the rental assistance voucher for TCL members so that apartments in preferred areas are available to our members. The goals of TCL are to help with rent and tenancy, and connect members with service providers and physical health care. TCL has a recovery-based philosophy of care, which means the support given promotes wellness and better long-term health outcomes for members.

Transitions to Community Living Local Barriers Committee (TCL LBC)
The Alliance TCL LBC is a way for TCL stakeholders to share the barriers members may face when transitioning to community living. This committee works to resolve barriers and may also raise system-wide issues to the state Barriers Committee to improve outcomes for TCL members or processes. Share a TCL barrier.
