Tailored Care Management

Tailored Care Management is a free service for eligible Alliance Health members. Care managers provide extra support for members to get and stay healthy. They use a whole-person approach when working with you. Your care manager will learn about your strengths and ask questions to understand your current needs. Your care manager will work with you and your healthcare providers to create a plan to address your goals and needs.

How can Tailored Care Management help me?

When you work with a Tailored Care Manager, you have a partner in managing your health, so you do not have to do it all on your own.

Your care manager will collect information to understand your health history and care needs. Then, you and your care manager, along with any other supports you may need, will create a care plan that outlines the services and supports you feel are needed to improve your health and wellness. This may be usual supports, such as finding a primary care doctor, or community resources to help with your needs such as access to housing or food. Your care manager will work with you to make sure your voice is heard, and your choices are respected throughout the process.

A Tailored Care Manager will be your main point of contact for all things Medicaid. They can also help you:

  • Find doctors who take Medicaid and have openings
  • Get needed equipment and prescriptions
  • Schedule free rides to appointments
  • Get and keep a job
  • Find resources for housing and food
  • Manage chronic (long term) conditions, like diabetes
How do I get started?

You have a choice of who you work with for Tailored Care Management. As well as Alliance there is also the option to get Tailored Care Management through some primary care providers (Advanced Medical Home Plus-AMH+) or some behavioral health/IDD/TBI service providers (Care Management Agencies-CMA). You can access a list of Tailored Care Management providers in the Alliance network.

To choose or change your Tailored Care Management provider or care manager please call our Member and Recipient Services Line at 800-510-9132 or download the necessary form.

What if I change my mind?

Members can choose to not have Tailored Care Management at any time. If a member chooses to not have Tailored Care Management, they can still get support through time limited Care Coordination or Innovations/TBI Waiver care coordination.
