  • Psychological Evaluation
  • Person Centered Plan (PCP)/Individualized Service Plan (ISP) with signed Service Order

Concurrent:

  • Service Authorization Request (SAR)
  • Updated Person Centered Plan (PCP)/Individualized Service Plan (ISP)

  • Maximum of 28 hours/week for 6 months
  • 1 unit = 15 minutes
