Change in Medical Records Request Process

Alliance Health’s contract with Reveleer has ended effective June 30, 2026. Accordingly, Reveleer is no longer authorized to conduct medical record retrieval activities on behalf of Alliance Health for HEDIS and other quality improvement initiatives.

Moving forward, all medical record requests will be managed directly by Alliance Health or through representatives identified and expressly authorized by Alliance Health. If you receive a request for medical records, please confirm that the request originates from Alliance Health or an authorized Alliance Health representative prior to disclosure and that it follows our established retrieval and submission processes.

We appreciate your continued partnership and support in providing timely access to medical records, which remains essential for accurate HEDIS reporting, quality measurement, and member care improvement.

If you have received, or later receive, a medical record request from Reveleer after June 30, 2026, please promptly contact Alliance Health before responding to the request.
