Initial:

  • Service Authorization Request (SAR)
  • Psychological/Psychiatric/Neuropsych Evaluation
  • Person Centered Plan (PCP) or Individual Support Plan (ISP) with signed Service Order
  • TBI Assessment requiring minimum to low supervision/support level

  • 30 hours/week for 6 months
