Change to Benefit Plan Services Look Up Tool
Effective January 1, 2026, there will be some changes to where to find prior authorization information on our Alliance Health website: https://www.alliancehealthplan.org/.
Our non-Medicaid funded services (to include reference documents, authorization submission requirements and authorization duration and limits) will be available on our Non-Medicaid Benefit Plan Lookup Tool at https://www.alliancehealthplan.org/services/.
Our Medicaid funded services (to include requirements for prior approval, and how to request prior approval) will be available on our Procedure Code Lookup Tool at https://www.alliancehealthplan.org/providers/procedure-code-lookup-tool/.
