Provider Networks is reaching out to licensed professionals (LP’s) in the network to highlight the importance of ensuring that your contact and profile information is accurate within the Alliance Practitioner Search tool. This search tool is used by members and other stakeholders to find practitioners within the network. All practitioners contracted with Alliance for services are required to update Alliance when there have been any changes in contact information including changes in referral status, names, office location, phone number, licensure, specialties, languages spoken, and/or agency affiliation.

The Practitioner Search Tool Review form can be used to review the information that is currently listed. If there are any updates that are necessary the LP can submit the form to the Provider Network Helpdesk at [email protected].
