The Patient Protection and Affordable Care Act requires employers to offer health insurance coverage to full-time employees and their dependents. Alliance Health must provide an annual statement to all employees eligible for coverage that describes the health insurance offered to them. The Internal Revenue Service (IRS) created Form 1095-C that serves as the statement that employers complete and distribute to each employee. As an employee who is eligible for health insurance coverage, you should receive a 1095-C, even if you declined the health plan offered to you.
On December 23, 2024, the President signed two bills aimed at simplifying employers’ Affordable Care Act (“ACA”) reporting requirements: the Paperwork Burden Reduction Act and the Employer Reporting Improvement Act.
The Paperwork Burden Reduction Act (the “Act”) limits the circumstances under which Form 1095-C must be provided to employees. Starting with the 2024 calendar year, instead of mailing Forms 1095-C to employees, the Act allows employers to furnish Forms 1095-C only upon the request of employees.
To access your 1095-C
Current employees:
- Login to UKG, under the Myself menu> select Benefits >1095-C.
- Once on the Form 1095-C page, in the year 2024 row >click 1095-C under the Form column. A PDF version of your 1095-C will populate for you to download and print.
Former employees:
Please submit a written request to the Human Resources department. Upon receipt of your request, we will provide your form electronically within 30 days, or by mail if preferred.
