Eligibility

Alliance can cover the MOUD formulary below for an individual if the pharmacy confirms the individual is:

  • Uninsured via checking COB (coordination of benefits) or underinsured (their insurance does not cover MOUD) AND
  • Resides in any of Alliance Health’s seven counties – Cumberland, Durham, Harnett, Johnston, Orange, Mecklenburg and Wake.

*Lower-dose strengths and FILMS are allowed for up to 7-day fill one time to support low-dose inductions.

NOTE to prescribers: buprenorphine 8 mg and BUP/NLX 8/2mg formulations cost the same or less per tablet as lower-dose strengths. Prescribing higher doses with multiple lower dose strength of BUP/NLX (2/0.5 or 4/1 mg) or buprenorphine 4 or 2 mg tablets significantly increases the cost of these prescriptions.
