  • Record supporting Severe Persistent Mental Illness (SPMI) and/or multiple hospitalizations/crisis episodes
  • Comprehensive Clinical Assessment (CCA)

Concurrent:

  • Service Authorization Request (SAR), Person Centered Plan (PCP) update

Authorization Duration and Limits Information Icon
