Clinician Directory Accuracy Survey coming soon
Alliance will begin to reach out via email to randomly selected providers who provide outpatient services beginning the week of Oct 25 in order to complete a yearly survey on the accuracy of the Clinician Directory.
If you are contacted please respond back with the information requested within the time frames requested. This process will include verifying information such as location, phone number, status of accepting new patients, and identification of languages spoken. We appreciate your responses to help keep the directory information up to date.
